　　【摘要】目的：探讨上颌单颌拔牙矫治安氏II类1分类错的疗效、适应证和临床意义。方法：对14例安氏II类1分类错合患者采用上颌单颌拔牙模式进行矫治，将矫治前后的X线头影测量数据进行对比。结果：①1-FH角减少11.03°，1-MP 角增加3.07°；②下颌综合长度增加3.30 mm；③上、下唇突度分别减小2.65 mm和1.64 mm。上述测量项目矫治前、后的均值之间，差异具有显著性(P＜0.05～0.001)。结论：上颌单颌拔牙模式适用于下切牙唇倾度和下唇突度较小的牙源性安氏II类1分类错患者。

【Abstract】Objective　To investigate the effects, indications and clinical significance of single maxillary extraction orthodontics used for class Ⅱ division 1 malocclusions.Methods　14 class Ⅱ division 1 patients were treated by single maxillary extraction orthodontics, the cephalometric data before and after treatment were analysed by statistical methods.Results　 After treatment ,①the angle 1-FH had decreased by 11.03degrees and the angle 1-MP had increased by 3.07degrees; ②the mandibular length had increased by 3.30 mm;③the upper and lower lip were retruded by 2.65 mm and 1.64 mm respectively. Statistical significance (P＜0.001～0.05) was found between the means before and after treatment.Conclusions　Single maxillary extraction orthodontics is suitable for class Ⅱ division 1 patients with less labial tipping of lower incisors and less lower lip protrusion.

　　有关上颌单颌拔牙矫治安氏II类1分类错?的研究报道目前尚不多见。我们试图通过分析对比此类病例矫治前后的变化，就采用上颌单颌拔牙模式矫治安氏II类1分类错?的疗效、适应证和临床意义进行探讨。
材料与方法
　　1．临床资料： 选择矫治完成的14例安氏Ⅱ类1 分类错合病例。其中，男4例，女10例，平均年龄(13.7±3.1)岁。所有患者矫治前均表现为前牙深覆盖和磨牙远中或偏远中关系；采用拔除双侧上颌第一双尖牙的拔牙模式进行正畸矫治。矫治后的牙?关系为：前牙正常覆?、覆盖，尖牙呈I类关系，磨牙呈完全远中关系。平均疗程为(20.9±4.3)个月。
　　2．研究方法：
　　（1）对本组14例矫治前后摄取的X线头颅侧位片由作者进行描绘和定点，使用计算机图形数字化仪进行测量及显示。测量标志点及项目见图1，2。
　　（2）统计学分析：使用SPSS 6.0统计软件对矫治前后的测量均值进行配对t检验，显著性水准为α＝0.05。
